Thank you for choosing Tri-State Mountain Neurology for your neurological needs. We require a referral from your primary care physician to be seen by our physicians. Also please verify that we take your insurance before scheduling an appointment. We want to make your first visit with us as smooth as possible. With that being said we will need certain information from you for our records. Please print out the forms below and have them filled out prior to your arrival. Please bring a list of your medications along with your insurance card. Completing the forms in their  their entirety will help to avoid any delays in your visit.
